HEDMAN WELLNESS SERVICES AGREEMENT

Hedman Wellness Services business address is located at 2955 New Center Point, #1008 Colorado Springs, CO 80922, (719) 960-0550. Hedman Wellness is a company that practices and engages in trauma-informed professional coaching, consulting and facilitation. Including Internal Family Systems, life, business, and personal coaching, consulting and facilitation. The coach, consultant and facilitator is:

INTERNAL FAMILY SYSTEMS
Internal Family Systems is an evidence based therapeutic healing model founded by Dick Schwartz, PhD in the 1980s. IFS helps to reintegrate the parts of ourselves that appear to be in conflict. IFS allows for a healthy, functioning whole self of an individual. IFS works to remedy the internal contradictions that arise from the complexity of life. It is a compassionate, client-led model that provides a non-shaming environment for all parts of an individual to be heard, healed, and integrated. It combines systems thinking with the view that the individual is made up of many different thoughts, feelings, needs and behaviors; each with its own unique viewpoint and qualities. IFS uses family systems theory to understand how these collections of subpersonalities are organized.

Any hours of consulting with Tara Hedman are not accredited or applicable toward any IFS certification. Tara Hedman cannot provide IFS-institute certification hours.

HEDMAN WELLNESS SERVICES AND PSYCHOTHERAPY
In addition to being a coach, consultant and facilitator, Tara Hedman is also an Unlicensed Psychotherapist in the state of Colorado with training and experience treating people with emotional challenges. While there are similarities between the services provided by Hedman Wellness and psychotherapy, they are very different activities and it is important that you understand the differences between them. Both our services and psychotherapy utilize knowledge of human behavior and motivation, behavioral change, and use the relationship between the coach, consultant, and facilitator/client, or therapist/patient, to encourage and influence change. The major differences lie in the goals, focus, and level of professional responsibility.

The focus of the services provided by Hedman Wellness is the development and implementation of strategies to reach goals set by the client. Through our services, we may address specific personal projects, life balance, job performance or satisfaction, spiritual growth, or general conditions in a client's life, relationships or profession. Hedman Wellness utilizes strategic planning, values clarification, brainstorming, motivational instruction and other techniques. The client's thoughts, behaviors, emotions and physical symptoms are examined and explored as relevant sources of information related to our goals.

In contrast, the primary focus of psychotherapy is the identification, diagnosis, and treatment of mental health "disorders". The goals of psychotherapy include alleviating symptoms, understanding and resolving the dynamics which create distress, modifying dysfunctional behaviors, and developing new strategies for coping with mental and emotional challenges.

Because of these differences, the roles of coach, consultant and facilitator, and psychotherapist can often be in conflict. Under most circumstances, it is ethically inappropriate for one person to play both roles with a client, whether concurrently or sequentially. This means that if either you or Tara Hedman, recognize that you have a struggle or challenge that would benefit more from psychotherapeutic intervention, Tara will refer you to appropriate resources.

Tara Hedman does not see clients under the age of 18.

CONFIDENTIALITY 
The coaching, consulting, and facilitating relationship, as well as all information (documented or verbal) that the client shares with the provider is kept private through HIPPA compliant service providers. However, please be aware that the coach/practitioner-client relationship is not considered a legally confidential relationship (like the medical and legal professions) and thus communications are not subject to the protection of any legally recognized privilege.

The coach, consultant, facilitator agrees not to disclose any information pertaining to the client without the client's written consent. The coach, consultant, facilitator will not disclose the client's name as a reference without the client's consent. Private information does not include information that: (a) was in the coach's possession prior to its being furnished by the client; (b) is generally known to the public or in the client's industry; (c) is obtained by the coach from a third party, without breach of any obligation to the client; (d) is independently developed by the coach without use of or reference to the client's private information; or (e) the coach is required by statute, lawfully issued subpoena, or by court order to disclose; (f) is disclosed to the coach and as a result of such disclosure the coach reasonably believes there to be an imminent or likely risk of danger or harm to the client or others; and (g) involves illegal activity. The coach, consultant, or facilitator is a mandatory reporter under her unlicensed psychotherapy status in the state of Colorado. The client also acknowledges his or her continuing obligation to raise any privacy questions or concerns with the coach in a timely manner.


Hedman Wellness Services uses external providers to enhance services including the Upheal platform. Upheal empowers practitioners to concentrate on their services by offering automated notes and analytics for client conversations. As a part of this process, Upheal handles protected health information for practitioners, adhering to HIPAA regulations as a Business Associate.

Hedman Wellness Services has signed a Business Associate Agreement (BAA) to protect data that is shared with Upheal. Under the BAA, Upheal adheres to regulations such as the HIPAA Security Rule and Privacy Rule. This ensures that electronic health information (ePHI) is safeguarded through appropriate administrative, physical, and technical measures, ensuring its confidentiality, integrity, and security. AS A HEDMAN WELLNESS CLIENT
As a Hedman Wellness client, if you would like to receive our services, you must agree to the following statements. As a Hedman Wellness client:

I understand that Tara Hedman is not providing me with psychotherapy services. Should I desire psychotherapy services, it is my responsibility to contact a separate mental health professional. I understand that Tara Hedman may suggest that I seek separate mental health services. I understand that I always retain the right to choose my service providers, which includes, if I so choose, requesting psychotherapeutic services from Tara Hedman. I further understand that if I should desire to receive professional mental health services from Tara Hedman, our Hedman Wellness relationship will cease, I will become a psychotherapy client, and I will never again be able to engage Tara Hedman's services as a coach, consultant and facilitator.

I understand that although Tara Hedman may be trained in a variety of skills she will not be acting in the role of counselor, psychotherapist, or any other mental health professional during our sessions. I further understand that the services offered at Hedman Wellness are not psychotherapy.

Tara Hedman is a Certified Trauma Care Practitioner, accredited by the Global Trauma Institute, LLC. As part of the certification, Tara Hedman is bound to uphold the Institute's Code of Ethics.

I understand that coaches, consultants and facilitators are not regulated by the Department of Regulatory Agencies, Division of Professions and Occupations, are not required to abide by the Colorado Mental Health Practices Act, C.R.S.12-245-101, et seq., and are not bound by any mental health professional ethical rules of confidentiality. However, I understand Tara Hedman will protect my confidential communications unless required by law to disclose such information.

I understand that Tara Hedman is available by telephone during most business hours. However, if she is in session with another client, I understand she will not pick up the phone. I may choose to leave a voice message. All phone calls will be returned within 48 business hours.

I understand that Tara Hedman is available to help and assist me in various aspects of my life and/or profession. However, I understand that I alone am responsible for the decisions and choices I make. I understand that the services at Hedman Wellness do not replace professional advice such as psychological, legal, financial, medical, tax, or other matters normally handled by other professionals. I understand that I may seek an opinion from another person regarding our conversation, at any time. 

I agree to continue seeing my regular doctor or mental health professional, and all medical or psychological decisions will be made explicitly between my health professional, counselor, and me.

Sexual intimacy is never appropriate in a professional relationship.

I understand that I alone am responsible for the decisions, choices, and advice I choose to take and the consequences of those decisions, choices, and advice I follow. I may choose to reject any suggestions, advice, or requests Tara Hedman makes.

I understand that coaching, consulting and facilitating may occur by in-person meetings or by scheduled phone or live video meetings that I am not permitted to record without written consent from Hedman Wellness Services.

I agree to pay Hedman Wellness $225.00/ 50 minutes for the services offered. I understand that Hedman Wellness does not take insurance and therefore a credit card payment is due at time of service.

Hedman Wellness is proud to support the non-profit organization Love146. For every session you purchase with Hedman Wellness, a portion of the proceeds are donated to Love146.

I understand that being under the influence of drugs or alcohol during the session will cause the session to be terminated early. The full charge will apply. Multiple sessions where the client is under the influence may end the service relationship.

I understand that all intensives, weekly, biweekly, and one-off sessions are charged on the day of service, and all sales are final.

I understand that should I cancel within 24 business hours of my appointment, or fail to show up for my scheduled appointment without notice ("no-show"), Hedman Wellness has a right to charge my credit card on file for the full amount of my session.

I understand that I can terminate this agreement at any time.

I certify that to the best of my knowledge, I do not have any medical or psychological condition or any physical issues, which would prohibit me from participating in Hedman Wellness sessions. Being 100% responsible for myself, I am open to receive and benefit from the sessions offered at Hedman Wellness. I am aware of and assume all risks inherent in participation in Hedman Wellness services. The risks may include, but are not limited to, physical, emotional, and mental responses, reactions, and changes.

I understand Tara Hedman provides non-emergency services by scheduled appointment only. If, for any reason, I am having an emergency, I will call 911, check myself into the nearest hospital emergency room, or call the National Suicide Prevention Lifeline at 1-800-273-TALK(8255). Hedman Wellness/Tara Hedman does not provide after-hours service without an appointment.

My signature below affirms that I have read and understand the preceding information, it has been provided to me verbally if I have no written language or am unable to read, and I agree to abide by the terms of the agreement.
